Saints' Cards are devotional collectibles in Mafia: The Old Country — small rectangular cards bearing the image and prayer of a Sicilian saint, each one kept at a roadside shrine across the island of Sicily.
There are 20 Saints' Cards in total. Collecting all 20 is required for The Collector trophy alongside Newspapers, Photographs, and Mystery Foxes.

Unlike most collectibles that are simply picked up from surfaces, Saints' Cards require interacting with the shrine — Enzo prays at the roadside altar and receives the card.
The shrine icon is distinct: on the full Sicily map when zoomed in, Saints' Card shrines appear with a chapel icon rather than the generic white collectible marker used for Newspapers and Locked Boxes. This makes scanning the map for remaining shrines straightforward once the card collection is started.
The first Saints' Card — Santa Barbara — is introduced in the Prologue: Tremori as a game-directed moment. After escaping the sulfur mine, Gaetano specifically instructs Enzo to stop and pray at the roadside shrine. This is the mechanic's tutorial and is nearly impossible to miss.
Two story-exclusive Saints' Cards are in areas that close after their chapter ends and cannot be accessed in Explore Mode:
18 Free Roam Saints' Cards are scattered across the roadsides, hillsides, coastal paths, cemeteries, caves, and village squares of Sicily. All are accessible at any time in Explore Mode after completing Chapter 3.
Saints' Cards provide no gameplay benefit — no stat boost, no Dinari, no Charm effect.
Each card contains a short prayer about its saint and their significance to the communities of early 1900s Sicily, adding texture to the world's religious culture.
Their sole mechanical purpose is to contribute to the Collector trophy.
