Mafia: The Old Country: Medallion Locations & Interactive Tracking Guide

In Mafia: The Old Country, Medallions are the most potent charms Enzo can equip on his Rosary. Unlike Beads, which offer utility and incremental boosts, Medallions provide game-changing passive abilities that can define your entire combat strategy.


There are only 5 Medallions in the game. Because you are restricted to equipping only one Medallion at a time, choosing the right one for your current mission is the ectremely important to surviving the escalating gang wars of Sicily.


Medallions are retrieved through exploration or by spending significant amounts of Dinari:


  • Four of the five Medallions are found as hidden collectibles within story chapters. They appear as small, shimmering golden statues.
  • The final Medallion must be purchased with Dinari from Pasquale's shop, located north of the San Celeste town square.


Since you can only carry one Medallion, you should swap them at any Traveler's Trunk or between Chapters based on your objective:


  • Assassinations: Equip L’Ombra. The increased detection time is critical for getting through the high-security areas of the Teatro Massimo or the Baron’s villa.
  • Combat: Equip Il Protettore. During the massive shootouts in Chapter 11 (Mattanza), that 15% damage reduction can be the difference between life and death on Old Country difficulty.
  • Duals & Brawls: Use Senza Pietà. If you are skilled at timing your parries, this Medallion allows you to remain aggressive indefinitely without running out of breath.


Because the four exploration-based Medallions are obtained in specific story chapters, missing them can leave you underpowered for the endgame. If you miss a statue, you must use Chapter Replay from the main menu to retrieve it.


To avoid this, all Medallion locations have been marked on our interactive Mafia: The Old Country game map and listed below.

Sant'AgataSicily

Found on the table next to the wardrobe in your room. It can be picked up during Chapter 1, before getting ready for the meal.


Effect:

  • Blesses you with a period of invulnerability when close to death.
San GiudaSicily

Found on a small crate close to Gennaro during Chapter 4.


Effect:

  • Guarantees that weapons looted from enemies will be full of ammo.
San Bernardo da CorleoneSicily

Found on a table in the basement of the building. It can be obtained while looking for the forger during Chapter 8, right after an ally shoots at you by mistake.


Effect:

  • Grants you the chance of being able to kill an enemy with one shot.
San CristoforoSicily

Found between some flower pots in front of a building's door. It can be obtained while escorting Isabella to the church during Chapter 10.


Effect:

  • Provides all of your guns greater ammo capacity.
San Francesco d'AssisiSicily

It can be bought in Pasquale's Store for 800 Dinari.
