Mafia: The Old Country: Medallion Locations & Interactive Tracking Guide

Medallions are the most powerful Charms in Mafia: The Old Country — single-slot passive abilities that define Enzo Favara's core playstyle in a way no Bead combination can match.


Only 5 Medallions exist in the base game, and only one can be equipped at a time, making the right choice for each mission genuinely consequential. Four are found as golden statues inside story chapters, and one is purchased from Pasquale's Store.

All 5 Medallions and Their Effects


Every Medallion falls into one of five Charm categories — Health, Loot, Combat, Inventory, and Stealth:


Sant'AgataHealth — Chapter 1: Famiglia. Left on Enzo's desk by Father Ciccone at the end of the chapter's first day.

  • Effect: grants a brief period of invulnerability when health drops critically low. The first Charm in the game is a reliable passive safety net for all difficulty levels.


San GiudaLoot — Chapter 4: Il Barone. Found on a small crate close to Gennaro after freeing him from the bandit camp.

  • Effect: any weapon looted from a defeated enemy is guaranteed to be full of ammunition. Dramatically reduces the need to manage ammo conservation when playing aggressively.


San Bernardo da CorleoneCombat — Chapter 8: Disgrazia. Found on a table in the basement of the building where the forger is located, right after an ally shoots at Enzo by mistake.

  • Effect: grants a chance of killing any enemy in a single shot. Considered the strongest base-game Medallion — on Hard and Old Country difficulty, where enemies absorb more damage, the one-shot kill proc changes the outcome of extended firefights. The Locked Boxes in the same basement room can be collected simultaneously.


San CristoforoInventory — Chapter 10: La Festa. Found between flower pots in front of a building's door while escorting Isabella to the church.

  • Effect: increases ammunition capacity across all guns. Most useful in the game's later chapters, where sustained firefights deplete standard carry capacity quickly.


San Francesco d'AssisiStealth — Pasquale's Store, 800 Dinari.

  • Effect: enhances knife blade durability — the blade lasts longer before dulling and requiring sharpening. The most valuable Medallion for stealth-focused players who pry open Locked Boxes frequently and rely on silent knife takedowns.


When to Swap Medallions


Medallions are swapped at any Traveler's Trunk — a storage item found at safehouses and certain chapter start points — or freely between chapters from the inventory screen. Recommended rotation by approach:


Health priority/survival on high difficulty: Sant'Agata throughout Chapters 1–7, replaced by San Bernardo da Corleone from Chapter 8 onward once the one-shot mechanic becomes most valuable in the larger enemy encounters of Chapters 9–13.


Loot and economy focus: San Giuda, through the mid-game, keeps enemy weapons fully loaded on pickup, reducing Dinari spent on ammunition at Pasquale's Store and supplementing Locked Box income.


Stealth runs: San Francesco d'Assisi from Pasquale's Store paired with the Fantasma Bead (quieter footsteps) creates the most complete silent approach loadout available in the base game.


DLC Medallions


Three additional Medallions are exclusive to special editions and pre-order bonuses and do not count toward the 5 base-game Medallions:

  • Lupara (Soldato pre-order pack, faster Lupara handling)
  • Immortale (Padrino Pack in the Deluxe edition, automatic bullet dodge chance)
  • Velocità (Gatto Nero Pack in the Deluxe edition, improved car handling and resistance).
Name Details
Sant'AgataSicily

Found on the table next to the wardrobe in your room. It can be picked up during Chapter 1, before getting ready for the meal.


Effect:

  • Blesses you with a period of invulnerability when close to death.
San GiudaSicily

Found on a small crate close to Gennaro during Chapter 4.


Effect:

  • Guarantees that weapons looted from enemies will be full of ammo.
San Bernardo da CorleoneSicily

Found on a table in the basement of the building. It can be obtained while looking for the forger during Chapter 8, right after an ally shoots at you by mistake.


Effect:

  • Grants you the chance of being able to kill an enemy with one shot.
San CristoforoSicily

Found between some flower pots in front of a building's door. It can be obtained while escorting Isabella to the church during Chapter 10.


Effect:

  • Provides all of your guns greater ammo capacity.
San Francesco d'AssisiSicily

It can be bought in Pasquale's Store for 800 Dinari.
