Vault Entrances are the gateways to the three Primordial Vaults of Borderlands 4 — the most demanding dungeons on Kairos, locked behind a regional Vault Fragment hunt and rewarding permanent Glide Pack upgrades alongside legendary loot on first completion.
Defeating all three Primordial Guardians unlocks the Nothing Left to Guard trophy and achievement. Every entrance location and its required Vault Fragments are cross-linked at exact coordinates on the interactive BL4 game maps.
Each Primordial Vault is sealed until 3 Vault Key Fragments from the same region are collected and inserted at the entrance altar.
Once all three fragments are in inventory, the game reveals the general area of the vault on the map — approach the entrance location to trigger the altar's appearance.
Fragments don't appear automatically on the map, but capturing Order Silos in each region reveals the general area of a nearby fragment via the communications balloon scan.

After the first clear, a Moxxi's Big Encore machine spawns outside each vault for infinite Boss respawns.
Each completed vault awards a distinct, permanent improvement to the Glide Pack traversal system:
